The Home-Based Primary Care (HBPC) Team Physician's is expected to be a geriatric medicine expert with knowledge and practice in geriatric medicine, transitions of care, and healthcare systems navigation. HBPC Team Physician's major duties and responsibilities include, but are not limited to:
Attends weekly in-person team meetings Serves as the collaborative physician for advance practice nurse primary care providers and as back-up for HBPC primary care providers as needed Provides assistance and consultation utilizing geriatrics principles Provides Consultation topics include identifying medical problems, defining medical management of problems, participating in the development of individual plans of care during interdisciplinary team meetings, determining the need for other subspecialty services, determining the need for and assisting in facilitating transitions of care, providing medical administrative support such as signing orders and forms, participating in HBPC committees and quality improvement projects, and being readily available by phone or in person for team members for collaboration when clinical or program issues arise Makes home visits for medical evaluation and treatment when clinically indicated and by request from team members Reviews and signs home health orders, chart reviews and contributes towards quarterly and annual plans of care Facilitates interprofessional collaboration and inter-systems communication in transitions of care Serves as a collaborator, consultant, and contributor to the HBPC team and program Participates in continuing medical education for license and certification and professional development. NOTE:

United States Citizenship:
Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. EDUCATION:
Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. LICENSURE:
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. RESIDENCY TRAINING:
Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. ENGLISH LANGUAGE PROFICIENCY:
Physicians appointed to direct patient-care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Physical requirements outlined below. Preferred

Must possess a current and valid driver's license Board Certification/Board Eligibility in Geriatric Medicine Reference:
VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements:
Moderate lifting (15-44 pounds);Light Lifting (15 lbs and under); Moderate Carrying (15-44 pounds); Light Carrying (15 lbs and under), use of fingers and both hands required; walking (up to 4 hours); standing (up to 6 hours); Kneeling (up to 1 hour); Repeated bending(up to 1 hour); ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13 to 16; Far vision correctable in one eye to 20/20 and to 20/40 in the other; Hearing (aid permitted); mental and emotional stability. Applicant must be willing to undergo a pre-employment physical that may involve a drug screening test. AND The individual must qualify to perform duties as a physician, have completed an accredited residency and/or possess experience which has qualified the individual to perform general duties and some specialized functions and procedures without supervision. Staff physicians may also include attendings who train physician residents assigned in facilities with residency training programs, and consultants who are capable of giving authoritative views and opinions on subjects in their field of medicine.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
