Behavioral Assessment Response
Team (BART) Community Liaison for Case Management at St Joseph Hospital in
Eureka, CA.
Full-time position; 80 hours
biweekly. 8-hour Day shift.
The Behavioral Assessment
Response Team (BART) Community Liaison position is responsible for developing,
implementing and evaluating the BART program as it relates to building
community relationships. Specifically,
the BART Community Liaison meets regularly with local mental health officials
and Law Enforcement to establish and maintain effective communication across
the continuum of care for the behavioral health patients. The BART Community Liaison requires
independent functioning, knowledge of assessment skills, and knowledge of
discharge planning in order to assist BART staff who provide direct patient
care. The BART Community Liaison identifies
community resources that may be needed and assists BART staff who provide
direct patient care to coordinate referrals and facilitate post-hospital care
to ensure a smooth transition from the hospital to an appropriate discharge.
? Ability to work closely with BART staff who
provide direct patient care while developing and improving the program.
? Efficiently communicates with customers and
employees of the organization and community partners, both in person and by
? Reads and comprehends written instructions,
correspondence, memorandums and medical records.
? Able to interpret correspondence, memorandums,
medical records and Mental Health regulations.
? Applies deductive and common sense reasoning
ability to problem-solve on a frequent basis.
Initiative and the ability to make independent judgments are required.
? Independent functioning as a representative of
Education: Graduate of an accredited
behavioral/mental health program.
Masters or Doctorate preferred.
Experience: Two years of acute care and/or
mental health specialty experience required or equivalent combination of
education/experience. Program management
experience preferred.
Training/Knowledge: Familiarity with legal and
ethical issues as related to psychiatric patients. Current working knowledge of community
resources for psychiatric patients.
Understanding of eligibility requirements for state and federal medical
benefit program for psychiatric patients, children and adults (i.e.: Medicare, Medi-Cal).
Licensure: None required.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
