Job Description:


We are looking for a detail-oriented and empathetic Case Manager, who has experience and is comfortable with covering ED and the inpatient units traveler will cover the inpatient floors/units, mainly at the Mission Viejo campus but may require travel to the Laguna Beach campus if coverage is needed.



Responsibilities:


  • Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered.
  • Handling case assignments, drafting service plans, reviewing case progress, and determining case closure.
  • Helping clients achieve wellness and autonomy.
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
  • Develop effective working relations and cooperate with the medical team throughout the entire case management process.
  • Record case information, complete accurately all necessary forms, and produce statistical reports.
  • Promote quality and cost-effective interventions and outcomes.
  • Assess and address motivational and psychosocial issues.


Job Requirement:


  • 2 years of Acute Care experience (can consider 1.5 years).
  • Experience with Utilization Review and Discharge Planning.
  • Experience with InterQual.
  • CA State RN License.
  • BLS card through AHA.

What is the schedule?


Shift:
Day 5x8-Hour (08:
00 - 16:
30).


Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
