Job Description:
We are looking for a detail-oriented and empathetic Case Manager, who has experience and is comfortable with covering ED and the inpatient units traveler will cover the inpatient floors/units, mainly at the Mission Viejo campus but may require travel to the Laguna Beach campus if coverage is needed.
Responsibilities:
- Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered.
- Handling case assignments, drafting service plans, reviewing case progress, and determining case closure.
- Helping clients achieve wellness and autonomy.
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
- Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
- Develop effective working relations and cooperate with the medical team throughout the entire case management process.
- Record case information, complete accurately all necessary forms, and produce statistical reports.
- Promote quality and cost-effective interventions and outcomes.
- Assess and address motivational and psychosocial issues.
Job Requirement:
- 2 years of Acute Care experience (can consider 1.5 years).
- Experience with Utilization Review and Discharge Planning.
- Experience with InterQual.
- CA State RN License.
- BLS card through AHA.

Estimated Salary: $20 to $28 per hour based on qualifications.
