The Registered Nurse will be responsible for providing high-quality clinical care, assessment, and support to patients in their homes, while collaborating closely with other healthcare professionals to ensure the delivery of coordinated and holistic care.
Responsibilities:
Patient Assessment and Care Planning:
Conduct comprehensive assessments of patients' health status and develop individualized care plans based on assessment findings, patient goals, and physician orders.
Monitor and evaluate patients' progress, symptoms, and response to treatment, adjusting care plans as needed to optimize outcomes.
Clinical Care and Procedures:
Provide skilled nursing care and perform clinical procedures including wound care, medication administration, IV therapy, and other treatments as prescribed by the physician.
Educate patients and caregivers on proper medication management, disease management, and self-care techniques to promote optimal health and independence.
Coordination of Care:
Collaborate with interdisciplinary team members including physicians, therapists, social workers, and home health aides to coordinate and deliver comprehensive care services.
Communicate effectively with healthcare team members to ensure continuity of care and facilitate timely interventions as needed.
Patient and Family Education:
Assess patients' and families' learning needs and provide education and support on disease management, medication compliance, lifestyle modifications, and other relevant topics to promote wellness and prevent complications.
Documentation and Compliance:
Maintain accurate and up-to-date clinical documentation in accordance with regulatory requirements, agency policies, and professional standards of practice.
Ensure compliance with all applicable laws, regulations, and guidelines governing home health care services.
Quality Improvement and Professional Development:
Participate in quality improvement initiatives, performance improvement activities, and peer review processes to enhance the delivery of care and outcomes for patients.
